LA Care Provider Home Health is a successful and fast-growing home health care company in West Los Angeles.
LA Care Provider Home Health is currently hiring responsible and independent individual with great leadership abilities,
strong clinical experience, and knowledgeable of home health policy procedures.
1. Registered Nurse
RESPONSIBILITIES AND DUTIES:
- Coordinate and provide case management following state and other regulations for all cases involving nursing and therapy services.
- Supervise staff and clinical services
- Responsible for the clinical operations and quality improvement programs of the agency.
- Knowledge of JCAHO, state, and federal regulations
- Plan, direct and participate in delivery of professional services through selection, orientation, training and evaluation of clinical staff.
- Participate in in-service programs, train, and teach other nursing personnel.
- Initial patient evaluation and re-evaluate patient's nursing needs on a regular basis.
- Develop and implement the nursing care for the patient under the direction of the physician.
- Experience with OASIS, ICD coding and writing treatment plans, physician's orders and various patient chart items.
- Provide nursing services, treatments, assessment and diagnostic procedures requiring specialized skills
- Assumes responsibility for each patient receiving nursing care.
- Follow physician's orders and produce accurate records of activities and services provided
- Direct, develop, recommend, implement, evaluate and interperet policies and procedures in compliance with state, and federal laws.
Requirements:
- Current, unencumbered state license to practice as a Registered Nurse (RN)
- Minimum of 3 to 5 years experience in a home health agency with at least 1 year experience in supervisory/management experience in home health
- Bachelor's degree prefered
- Demonstrated evidence of self-direction and strong organizational skills
- Demonstrated evidence of excellent interpersonal skills with patients and staff
- Must have physical abilities to adequately perform described duties.
- Able to travel to patient's homes, dependable car required for home visits.
- After hours on-call required as needed.

2. Registered Nurse

- Familiar with OASIS requirements as it relates to physician order, care plan/assessment, medication, etc.
